VA Skin Diseases DBQ (Dermatitis, Psoriasis)

Skin conditions claimed for VA disability, from dermatitis, eczema and psoriasis to acne, chloracne, urticaria, vitiligo, hyperhidrosis, bullous disorders and skin cancers, are documented on this 12-page questionnaire. Two measurements carry most of the form: the percentage of total and exposed body area affected, and how long the Veteran used corticosteroids, immunosuppressives or other systemic treatments in the past 12 months.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XI examiner's certification. The Veteran completes no part of it.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it.
Who pays
VA does not pay or reimburse any cost of completing the skin diseases DBQ; any fee is between the Veteran and the provider.

Good to know

  • Affected area is estimated separately for total body and for exposed areas (face, neck and hands) in bands: under 5, 5 to under 20, 20 to 40, and over 40 percent.
  • Each medication's total use in the past 12 months is marked under 6 weeks, 6 weeks or more but not constant, or constant or near-constant, along with route and condition treated.
  • Burns, and any skin condition causing scarring or disfigurement of the head, face or neck, require the Scars/Disfigurement DBQ.
  • The form asks whether the condition is active but without visible lesions at the exam, so a flare-free visit does not erase the record of the condition.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What if the Veteran's skin is clear on the day of the VA skin exam?

Item 4A asks whether the Veteran has a skin condition currently without visible characteristic lesions at the time of examination. The history in Section II and the treatment records in Section III, including medication duration over the past 12 months, then document the condition's course between flares.

Which treatments are recorded on the skin diseases DBQ?

Section III lists corticosteroids and other immunosuppressives, antihistamines, retinoids, sympathomimetics and other medications with route and duration, plus non-drug treatments in the past 12 months such as UVB phototherapy, each with the condition treated and date of the most recent treatment.
