VA Seizure Disorders (Epilepsy) DBQ

Veterans claiming epilepsy or another seizure disorder for VA compensation use this 7-page questionnaire. The clinician selects the seizure type, from tonic-clonic and absence seizures to Jacksonian, focal, diencephalic and psychomotor epilepsy, confirms the diagnosis and any witnessed seizure, and then counts major and minor seizures over the past 2 years and 6 months with their average frequency. Seizure frequency is the core of the form.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section IX examiner's certification. Witnesses to seizures are described by the examiner, including their relationship to the Veteran.

Submitting and payment

How to submit it
The Veteran receives the signed DBQ and uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it.
Who pays
VA will not pay or reimburse the cost of completing the seizure disorders DBQ; any fee is between the Veteran and the provider.

Good to know

  • Minor seizures are counted over 6 months; if 2 or more, the average is given as 0 to 4, 5 to 8, 9 to 10, or more than 10 per week.
  • Major seizures are counted over 2 years and the past year, then averaged, for example at least 1 in 6 months or at least 1 a month.
  • Epilepsy with a psychotic, psychoneurotic or personality disorder also requires the appropriate Mental Disorders questionnaire.
  • Scars that are painful, unstable, 39 square cm or larger, or on the head, face or neck need the separate scars questionnaire; smaller ones are measured on this form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a seizure need to be witnessed for the VA epilepsy DBQ?

Item 2E asks whether the Veteran has had a witnessed seizure and, if so, to describe it, including the witnesses' relationship to the Veteran. Separate items ask whether the diagnosis has been confirmed and whether there is a confirmed diagnosis of epilepsy with a history of seizures.

What tests does the seizure disorders DBQ record?

Section VI lists MRI, CT, cerebrospinal fluid examination, EEG and neuropsychologic testing, each with date and results. Results in the medical record that reflect the current seizure disorder do not need to be repeated.
