VA Rectum and Anus DBQ (Hemorrhoids, Fistula)

Hemorrhoids, anorectal fistulas and abscesses, rectal or anal stricture, dyssynergic defecation, impaired sphincter control, rectal prolapse and pruritus ani are documented for VA disability claims on this 7-page questionnaire. The clinician grades each condition against VA's criteria, such as persistent bleeding with anemia or thrombosis episodes per year, number of fistulas, degree of luminal narrowing, frequency of fecal incontinence, prolapse reducibility, and the response to a prescribed bowel program.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XV examiner's certification. The Veteran fills in no part of the form.

Submitting and payment

How to submit it
The signed DBQ is returned to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may file it for them.
Who pays
VA does not pay or reimburse the cost of completing the rectum and anus DBQ; any fee is between the Veteran and the provider.

Good to know

  • If anemia from hemorrhoids is checked, the form requires at least a hemoglobin or hematocrit value in the diagnostic testing section.
  • Fecal incontinence is graded by frequency and pad use, from less than once every six months up to more frequent episodes, along with any prescribed bowel program.
  • A Veteran with an ostomy also needs the Intestinal Conditions DBQ in addition to this rectum and anus questionnaire.
  • Section IX allows the rectal exam to be marked not performed, with the reason given, or normal or abnormal with a description.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How are hemorrhoids graded on the VA rectum and anus DBQ?

External and internal hemorrhoids are graded separately. Options include persistent bleeding, anemia (with a hemoglobin or hematocrit value), three or more thrombosis episodes per year, and, for internal hemorrhoids, continuous prolapse with thrombosis or prolapse with fewer episodes.

What does the form ask about anal fistula?

Section IV grades fistulas as one, two or more simultaneous, or more than two constant or near-constant, each with drainage, pain or abscess. Item 4B asks whether the fistula is refractory to medical and surgical treatment.
