VA Peritoneal Adhesions DBQ (Abdominal Adhesions)

Abdominal adhesions from surgery, trauma, infection or inflammatory disease such as chronic cholecystitis or Crohn's disease are documented for VA disability claims on this 4-page questionnaire. The clinician names the cause and the organs involved, marks the adhesions asymptomatic or symptomatic, and records diarrhea, constipation, colic, vomiting, pain, dietary modification and any persistent partial bowel obstruction, plus hospitalizations for adhesions in the past 24 months.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section IX examiner's certification. The Veteran completes no part of the form.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it instead.
Who pays
VA does not pay or reimburse any cost of completing the peritoneal adhesions DBQ; any fee is between the Veteran and the provider.

Good to know

  • For persistent partial bowel obstruction, the examiner states whether it is inoperable, refractory to treatment, and requires TPN for obstructive symptoms.
  • Recurrent obstruction counts when the health record shows hospitalization at least once a year; checking it also requires the hospitalization section with admission dates and facility.
  • Organs affected by adhesions, such as stomach, gallbladder, liver, intestines or pancreas, each need the matching digestive questionnaire.
  • The Intestinal Conditions DBQ refers examiners to this form when adhesions follow small or large intestine surgery.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
