VA Parkinson's Disease DBQ (Parkinsonism Form)

Veterans claiming Parkinson's disease or another parkinsonism, including progressive supranuclear palsy, multiple system atrophy, corticobasal degeneration, vascular, drug-induced or Lewy body disease, use this 10-page VA questionnaire. The clinician rates each motor sign by side and severity, from facial masking and stooped posture to tremor, rigidity and gait, then documents chewing, swallowing, bowel, bladder, speech, smell, taste, sleep, sexual and mental effects, because VA rates each manifestation separately.

Who completes it

The Veteran's treating or examining clinician completes Sections 1 to 13 and signs the Section 14 examiner's certification. The Veteran completes no part of the form.

Submitting and payment

How to submit it
The signed DBQ is returned to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may also submit it.
Who pays
VA will not pay or reimburse the cost of completing the Parkinson's DBQ; any fee is between the Veteran and the provider.

Good to know

  • Motor manifestations in Section 4 are rated mild, moderate or severe for each side: hypomimia, stooped posture, balance, bradykinesia, tremor, rigidity, weakness, reflexes, gait and atrophy.
  • Swallowing dysfunction must be documented by barium swallow, CT or EGD, with the study date given in the Remarks section.
  • If sleep apnea-like symptoms are due to Parkinson's or its treatment, the form states a sleep study is not indicated.
  • Depression, cognitive impairment or dementia due to Parkinson's requires a Mental Disorders questionnaire, as does dementia with Lewy bodies.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Why does the Parkinson's DBQ ask about smell and taste?

The form documents each manifestation separately, so Section 7 records speech changes, loss of smell, loss of taste, sleep disturbance and sexual dysfunction alongside the motor signs. Reporting each one that applies gives VA a full picture rather than only tremor or gait.

Does this form cover drug-induced parkinsonism?

Yes. Section 1 lists Parkinson's disease, parkinsonism of known or unknown type, progressive supranuclear palsy, multiple system atrophy (Shy-Drager syndrome), corticobasal degeneration, vascular parkinsonism, dementia with Lewy bodies and drug-induced parkinsonism, each with an ICD code and date of diagnosis.
