VA Pancreas Conditions DBQ (Chronic Pancreatitis)

Chronic pancreatitis, total or partial pancreatectomy, pancreas transplant and pancreatic neoplasms are documented for VA disability claims on this 8-page questionnaire. The clinician records ongoing treatment for pain or complications, hospitalizations for abdominal or mid-back pain, maldigestion requiring enzyme supplements, diabetes from pancreatic insufficiency, post-pancreatectomy symptoms such as post-prandial syncope or vomiting, and EUS, ERCP, MRCP, CT and amylase or lipase results.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section IX examiner's certification. The Veteran fills in no part of the form.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it.
Who pays
VA does not pay or reimburse the cost of completing the pancreas DBQ, so any fee is between the Veteran and the provider.

Good to know

  • Appropriate diagnostic studies must confirm that abdominal pain results from pancreatitis, and the diagnosis itself must be confirmed by laboratory and clinical studies.
  • After total or partial pancreatectomy, items 3A and 3B are completed even if the Veteran does not have pancreatitis.
  • Diabetes from pancreatic insufficiency needs the Diabetes Mellitus DBQ; vitamin A, E or K deficiency triggers the Eye, Peripheral Nerves or Hematologic questionnaire.
  • Hospitalizations for pancreas complications in the past 24 months, other than pancreatectomy or transplant, are listed with admission dates, facility and reason.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
