VA Osteomyelitis DBQ (Bone Infection Disability Form)

Veterans claiming osteomyelitis for VA compensation use this 7-page questionnaire to document the infection's history and current activity. The clinician records where the initial infection occurred, from pelvis and vertebrae to long bones, fingers and toes, medical and surgical treatment with dates, whether the condition is active, inactive or resolved, the number and dates of recurrences, and findings such as involucrum, sequestrum, a discharging sinus or amyloidosis.

Who completes it

The Veteran's treating or examining clinician completes the form, including the dominant hand, and signs the Section XII examiner's certification. The Veteran does not complete any section.

Submitting and payment

How to submit it
The Veteran receives the signed DBQ and uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may file it on their behalf.
Who pays
VA will not pay or reimburse any cost of completing the osteomyelitis DBQ; any fee is arranged between the Veteran and the provider.

Good to know

  • Recurrences after the initial infection are counted from 1 to 5 or more, each with its date and site, which VA uses to judge how chronic the condition is.
  • Decreased joint function or motion from osteomyelitis or its treatment requires the questionnaire for each affected joint or spinal segment.
  • An amputation due to osteomyelitis also requires the Amputations questionnaire, and anemia from chronic infection needs CBC results in Section IX.
  • Test items include bone scan, x-ray, MRI, CBC, CRP, ESR, blood culture, and bone biopsy and culture, each with date and results.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What osteomyelitis findings does the VA DBQ ask about?

Item 4A lists involucrum, sequestrum, a discharging sinus, amyloidosis secondary to chronic infection, anemia, other constitutional symptoms, and decreased joint function or range of motion. Item 4B covers symptoms such as pain, swelling, tenderness, erythema, warmth and malaise, each described by the examiner.

Does osteomyelitis of the jaw go on this form?

Yes, along with the Oral and Dental DBQ. The dental questionnaire directs examiners to complete the Osteomyelitis questionnaire for osteomyelitis or osteoradionecrosis of the mandible, so both forms document the jaw infection and any tooth or bone loss it caused.
