VA Rheumatoid Arthritis DBQ (Non-Degenerative)

Rheumatoid arthritis, psoriatic arthritis, spondyloarthropathies, infectious and crystalline arthritis, and decompression illness are documented for VA disability claims on this 8-page questionnaire. The clinician records medications, weight loss and anemia from the arthritis, which joints and body systems are involved, and how often non-incapacitating and incapacitating exacerbations occur, then answers four overall severity questions in items 5C to 5F.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XII examiner's certification. Each affected joint and body system is evaluated on its own DBQ as well.

Submitting and payment

How to submit it
The signed DBQ is returned to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may file it instead.
Who pays
VA does not pay or reimburse the cost of completing this DBQ; any fee is settled between the Veteran and the provider.

Good to know

  • Multi-joint arthritis means 2 or more joints as an active process, excluding post-traumatic arthritis and gout; degenerative arthritis is documented on the joint-specific DBQs.
  • Incapacitating exacerbations are counted per year (0 to 4 or more) with total incapacitation over 12 months, from under 1 week to 6 weeks or more.
  • Weight loss is measured against baseline, the average weight for the 2 years before onset, and anemia from the arthritis needs a CBC in Section IX.
  • Lab items include ESR, CRP, rheumatoid factor, anti-CCP, ANA and anti-DNA antibodies, plus joint aspiration or synovial fluid analysis and biopsy.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a VA DBQ for rheumatoid arthritis?

Yes. Rheumatoid arthritis is documented on the Non-Degenerative Arthritis DBQ, which VA lists as Arthritis under rheumatological conditions. It also covers psoriatic arthritis, spondyloarthropathies, infectious arthritis, crystalline and other autoimmune arthropathies, and decompression illness, formerly called caisson disease.

Do individual joints need separate DBQs with this form?

Yes. Section III asks which joints are involved, from the cervical spine to the feet, and the form says to complete the appropriate questionnaire for each identified joint. Involvement of the eyes, skin, kidneys, lungs, heart or other systems likewise needs that system's questionnaire.
