VA Muscle Injuries DBQ (Shrapnel, Gunshot Wounds)

Penetrating muscle injuries such as gunshot and shell fragment wounds, plus compartment syndrome and rhabdomyolysis, are documented for VA disability claims on this 12-page questionnaire. The clinician identifies each injured muscle group using VA's groups I to XXIII, describes entrance and exit scars, fascial defects and loss of muscle substance, records cardinal signs such as loss of power and lowered fatigue threshold, tests strength against the uninjured side, and notes retained metallic fragments on x-ray.

Who completes it

The Veteran's treating or examining clinician completes the form, including the dominant hand, and signs the Section XII examiner's certification. The Veteran completes no part of it.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may also submit it.
Who pays
VA does not pay or reimburse any cost of completing the muscle injuries DBQ; any fee is between the Veteran and the provider.

Good to know

  • Non-penetrating injuries such as strains, torn Achilles tendon or quadriceps, or atrophy from a joint or nerve injury go on the joint or nerve questionnaire instead; compartment syndrome stays on this form.
  • Strength is tested only for the affected muscle groups and the corresponding uninjured side, on the 0 to 5 scale.
  • Multiple muscle injuries should all be assessed on one form if possible, and a peripheral nerve injury from the same wound also needs the Peripheral Nerves DBQ.
  • Item 9B asks for x-ray evidence of retained shell fragments or shrapnel, or minute scattered foreign bodies showing the explosive effect of the missile, by muscle group and side.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which muscle injuries belong on the VA Muscle Injuries DBQ?

Mainly penetrating injuries, such as gunshot or shell fragment wounds, plus compartment syndrome. The form also asks about diaphragm rupture with herniation, extensive muscle hernia, facial muscle injury and rhabdomyolysis. Strains and tendon tears without a foreign object entering the muscle are rated on the relevant joint questionnaire.

What are the cardinal signs of muscle disability on this form?

Item 4D lists loss of power, weakness, lowered threshold of fatigue, fatigue or pain, impairment of coordination and uncertainty of movement. For each sign checked, the examiner records the side, the muscle group from I to XXIII if possible, and how often and how severe it is.
