VA Kidney Conditions DBQ (Nephrology Disability Form)

Renal dysfunction, diabetic nephropathy, kidney stones, glomerulonephritis, cystic kidney disease, kidney removal and transplant are documented for VA disability claims on this 9-page questionnaire. The clinician establishes renal dysfunction using GFR and urine findings sustained for 3 months, then records dialysis, structural abnormalities, stone treatment and colic, recurrent infections, transplant eligibility and the supporting lab results. VA rates kidney conditions from these findings.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XII examiner's certification. The Veteran completes no part of it.

Submitting and payment

How to submit it
The signed DBQ goes back to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it on the Veteran's behalf.
Who pays
VA does not pay or reimburse any cost of completing the kidney DBQ; any fee is between the Veteran and the provider.

Good to know

  • VA counts renal dysfunction when, for 3 consecutive months in the past year, GFR is below 60, or 60 to 89 with casts, structural abnormalities or proteinuria.
  • Two or more lab results in the past 12 months, at least 3 months apart and uncontradicted, are accepted as showing dysfunction persisted for 3 months, so repeat testing is not required.
  • Transplant eligibility means kidney function has declined enough that a transplant is or would be necessary; placement on a transplant list is not required for VA compensation.
  • Hypertension or heart disease caused by kidney disease needs the Hypertension or Heart Conditions DBQ, and voiding dysfunction from a kidney neoplasm needs the Urinary Tract questionnaire.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the VA kidney DBQ accept eGFR?

Yes. The form says GFR, estimated GFR and creatinine-based approximations of GFR are accepted for evaluation when appropriate and calculated by a medical professional. Item 9C records GFR results with dates, and item 9D asks about ACR of 30 mg/g or more and RBC, WBC or granular casts over 3 months.

Are kidney stones covered by the kidney DBQ?

Yes. Section IV covers kidney and ureteral stones: location, whether they cause ureteral stricture, treatment for recurrent stones (diet, drug therapy or procedures and how many per year), complications such as hydronephrosis, infection or impaired function, and whether colic attacks are occasional or frequent.
