VA Intestinal Conditions DBQ (IBS, Crohn's, Colitis)

Irritable bowel syndrome, Crohn's disease, ulcerative colitis, diverticular disease, celiac disease, bowel resections, bariatric surgery complications and intestinal fistulas are documented for VA disability claims on this 11-page questionnaire. Each diagnosis checked in Section I points the clinician to the sections to complete, which capture defecation-related pain frequency, treatment level, hospitalizations, diarrhea episodes, nutrition support, and endoscopy, imaging and lab results.

Who completes it

The Veteran's treating or examining clinician completes the sections that match each diagnosis and signs the Section XIX examiner's certification. The Veteran fills in no part of the form.

Submitting and payment

How to submit it
The Veteran receives the signed DBQ and uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may file it instead.
Who pays
VA will not pay or reimburse the cost of completing the intestinal DBQ, so any fee is settled between the Veteran and the provider.

Good to know

  • IBS and functional digestive disorders are graded by abdominal pain related to defecation over the previous 3 months: none, at least once, at least 3 days a month, or at least 1 day a week.
  • Inflammatory bowel disease must be confirmed by endoscopy or radiologic studies, and celiac disease by serum antibody testing or endoscopy with biopsy.
  • IBD treatment is tiered: oral or topical agents, outpatient immunosuppressants or biologics, unresponsive to treatment, or hospitalization at least once a year.
  • Peritoneal adhesions after intestinal surgery need the separate Peritoneal Adhesions questionnaire, and functional disorders affecting the esophagus need the Esophageal Conditions DBQ.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does IBS go on the VA intestinal conditions DBQ?

Yes. Irritable bowel syndrome is the first listed diagnosis and is documented in Section III with the frequency of defecation-related abdominal pain over the previous 3 months and signs such as changes in stool frequency or form, straining or urgency, bloating, distention and mucorrhea.

What does the form ask about bowel resection?

Section VIII covers small intestine resection, short bowel syndrome, mesenteric ischemia and celiac malabsorption, with diarrhea episodes, undernutrition, anemia and supplementation or TPN needs. Section IX covers large intestine resection, including partial or total colectomy, permanent colostomy, loss of the ileocecal valve and dehydration needing IV fluids.

How is diverticulitis documented?

Section V asks whether diverticular disease is asymptomatic, managed by diet and medication, or required hospitalization in the past 12 months, with dates and reasons such as abdominal distress, fever or leukocytosis, and complications including hemorrhage, obstruction, abscess or peritonitis.
