VA Hip and Thigh Conditions DBQ

Hip osteoarthritis, hip replacement or resurfacing, trochanteric pain syndrome, femoroacetabular impingement, avascular necrosis and femur fractures are documented for VA disability claims on this 13-page questionnaire. The clinician measures flexion, extension, abduction, adduction and rotation on both hips, estimates further loss after repeated use and during flare-ups, and records ankylosis, femur malunion or nonunion, leg length discrepancy, surgery residuals and assistive devices.

Who completes it

The Veteran's treating or examining clinician completes every section and signs the Section XIV examiner's certification. Flare-ups and functional loss are recorded in the Veteran's own words by the examiner in Section II.

Submitting and payment

How to submit it
The signed DBQ is given to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may file it for them.
Who pays
VA does not pay or reimburse the cost of completing the hip and thigh DBQ, so any fee is arranged between the Veteran and the provider.

Good to know

  • Normal endpoints are printed on the form: flexion 125, extension 30, abduction 45, adduction 25, external rotation 60 and internal rotation 40 degrees.
  • The form asks whether limited adduction prevents crossing the legs, a functional question VA uses alongside the degree measurements.
  • Leg length is measured in inches to the nearest quarter inch or in centimeters, from the anterior superior iliac spine to the internal malleolus.
  • Total hip replacement residuals are graded as none, moderately severe or markedly severe weakness, pain or limitation of motion, with the surgery date.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the VA hip DBQ cover hip replacement?

Yes. Hip joint replacement and resurfacing are listed diagnoses, and Section VII records the surgery date and residuals, graded from none to markedly severe weakness, pain or limitation of motion. If femur impairment also causes a knee disability, the form directs the examiner to complete the knee questionnaire too.

Must imaging be repeated for hip arthritis on this DBQ?

No. Degenerative or post-traumatic arthritis must be confirmed by imaging once, but after it is documented, even in the past, VA requires no further imaging even if the arthritis has worsened. Section XI records the side, test type, date and results.
