VA Hernias DBQ (Inguinal, Femoral, Abdominal)

Inguinal, femoral, umbilical, ventral and incisional hernias claimed for VA disability are documented on this 8-page questionnaire. For each type the clinician records surgery dates and type, whether a current or recurrent hernia is present and when it was documented, whether it is repairable or irreparable and why, the size of an irreparable hernia, and whether it causes pain with daily activities, bending, stairs or walking.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section X examiner's certification. The Veteran fills in no part of it.

Submitting and payment

How to submit it
The Veteran receives the signed DBQ and uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it instead.
Who pays
VA will not pay or reimburse any cost of completing the hernia DBQ; any fee is settled between the Veteran and the provider.

Good to know

  • Hiatal hernia is not rated here; the form directs examiners to the Esophageal Disorders questionnaire instead.
  • Irreparable hernias are sized as smaller than 3 cm, 3 cm up to 15 cm, or 15 cm or greater in one dimension, with the date and source of the measurement.
  • Deciding repairability should weigh current medical guidance, records already classifying the hernia, and significant contraindications to surgery, and the date and source of that determination are required.
  • Pain from an irreparable hernia is recorded by activity and side, along with whether it has lasted 12 months or more; no specific diagnostic testing is required.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What makes a hernia irreparable on the VA hernia DBQ?

The examiner considers whether that type of hernia can usually be repaired surgically, any medical records that already classify it as repairable or irreparable, and significant medical contraindications to surgery. The form asks for the date and source of the irreparable determination and an explanation for each side.

Can the hernia DBQ be used for an incisional hernia after surgery?

Yes. Section V covers umbilical, ventral, incisional and other hernias, with the type of hernia, surgery dates and types, whether a current or recurrent hernia is documented, and repairability, size and activity-related pain for any irreparable hernia. Scars from the repair need the dermatological questionnaire.
