VA Liver Conditions DBQ (Hepatitis, Cirrhosis)

Hepatitis A, B and C, autoimmune and drug-induced hepatitis, NASH, cirrhosis, primary biliary and sclerosing cholangitis, hemochromatosis and liver transplant are documented for VA disability claims on this 9-page questionnaire. The clinician records symptoms such as fatigue, anorexia and weight loss, cirrhosis complications with dates in the past 24 months, any MELD score, abscess or transplant details, and imaging, biopsy and lab results including hepatitis C genotype and viral titers.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XII examiner's certification. The Veteran does not complete any section.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it too.
Who pays
VA does not pay or reimburse the cost of completing the liver DBQ; any fee is between the Veteran and the provider.

Good to know

  • Liver diagnoses must be documented by serologic testing, abnormal liver function tests, or abnormal biopsy or imaging; results already in the record need not be repeated.
  • Weight loss is graded as minor (10 to 20 percent of baseline) or substantial (over 20 percent), each sustained for three months with reduced self-care or work tasks.
  • Cirrhosis requires liver dysfunction confirmed by biochemical studies, imaging or biopsy, such as hyponatremia, thrombocytopenia or coagulopathy.
  • Variceal hemorrhage, portal gastropathy and hepatic encephalopathy are recorded with dates of episodes in the past 24 months.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the VA liver DBQ cover hepatitis C?

Yes. Hepatitis C is a listed diagnosis. The examiner records symptoms, continuous medications, and whether parenteral antiviral and immunomodulatory therapy are both medically contraindicated, which routes the case to the cirrhosis section. Section VII captures RIBA, hepatitis C genotype and viral titers, AST, ALT, bilirubin, INR and biopsy results.

What transplant details does the liver DBQ ask for?

Section VI asks whether the Veteran is eligible for transplant surgery and the date a physician made that determination, whether the Veteran is awaiting a transplant, and, if transplanted, the surgery date, hospital admission and discharge dates, and current signs and symptoms.
