VA Hand and Finger Conditions DBQ

Trigger finger, Dupuytren's contracture, mallet finger, gamekeeper's thumb, joint replacements, arthritis and ankylosis of the digits are documented for VA claims on this 17-page questionnaire. The clinician measures MCP, PIP and DIP motion for every finger and the thumb on both hands, gaps between thumb pad and fingers and between fingertips and palm, grip strength and atrophy, then estimates loss with repeated use and flare-ups.

Who completes it

The Veteran's treating or examining clinician completes all sections for both hands and signs the Section XII examiner's certification. The Veteran's description of flare-ups and functional loss is recorded by the examiner in Section II.

Submitting and payment

How to submit it
The signed DBQ goes back to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it on their behalf.
Who pays
VA does not pay or reimburse any cost of completing the hand and finger DBQ, so any fee is arranged between the Veteran and the provider.

Good to know

  • Gaps are measured in centimeters: thumb pad to fingers, and index or long fingertip to the proximal transverse crease of the palm on maximal flexion.
  • The form defines the position of function, with wrist dorsiflexed 20 to 30 degrees and MCP and PIP joints flexed to 30 degrees; only ankylosis in that position counts as favorable.
  • Item 9C asks whether degenerative or post-traumatic arthritis is documented in multiple joints of the same hand; once arthritis is shown on imaging, VA requires no repeat imaging.
  • Examiners should test the opposite hand unless contraindicated and address pain on active and passive motion, explaining any test that cannot be performed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the VA hand DBQ cover trigger finger and Dupuytren's contracture?

Yes. Both are listed diagnoses, along with swan neck and boutonniere deformities, mallet finger, gamekeeper's thumb, chronic collateral ligament instability, volar plate injury, MCP or PIP prosthetic replacement, ankylosis, several forms of arthritis, gout, tendinitis and tenosynovitis. Each is marked right, left or both.

How is grip strength recorded on the hand and finger DBQ?

Section IV rates hand grip on each side on a 0 to 5 scale, from no muscle movement to normal strength. If strength is reduced, the examiner states whether it is due to the claimed condition, then records any muscle atrophy with circumference measurements of the normal and atrophied sides.
