VA Gallbladder and Biliary Tract Conditions DBQ

Gallbladder removal, chronic cholecystitis or cholelithiasis, biliary strictures, bile duct injury, cholangitis and other biliary tract conditions are documented for VA disability claims on this 7-page questionnaire. The clinician records clinically documented right upper quadrant attacks in the past 12 months, symptoms after cholecystectomy including diarrhea frequency, injury residuals, hospitalizations in the past 24 months, and imaging and lab results.

Who completes it

The Veteran's treating or examining clinician completes Sections I to VIII and signs the Section IX examiner's certification. The Veteran completes no part of the form.

Submitting and payment

How to submit it
The Veteran receives the signed DBQ and uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it for them.
Who pays
VA does not pay or reimburse any cost of completing the gallbladder DBQ; any fee is arranged between the Veteran and the provider.

Good to know

  • Attacks count only when clinically documented: the form asks for 1 or 2 versus 3 or more in the past 12 months, whether they included nausea or vomiting, and each date.
  • Post-cholecystectomy diarrhea is graded as one to two watery bowel movements a day or chronic diarrhea with three or more a day.
  • Recurrent obstruction counts when the health record shows hospitalization at least once a year, which also triggers the 24-month hospitalization item.
  • Imaging options include EUS, ERCP, transhepatic cholangiogram, MRI or MRCP, HIDA scan and CT; existing results reflecting the current condition need not be repeated.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is gallbladder removal rated on the VA gallbladder DBQ?

Yes. Cholecystectomy is the first diagnosis listed, and item 3C asks about symptoms afterward: none, intermittent or recurrent post-prandial or nocturnal abdominal pain, and diarrhea graded by number of watery bowel movements per day. Other gallbladder surgeries go in item 3D with dates and facility.

What lab tests does the gallbladder DBQ list?

Item 6B lists alkaline phosphatase, bilirubin, WBC, amylase and lipase, each with date and results, plus space for other tests. The examiner then states how any abnormal result relates to the diagnosed condition.
