VA Fibromyalgia DBQ (Disability Benefits Form)

Veterans claiming fibromyalgia, also called fibrositis or primary fibromyalgia syndrome, use this 7-page VA questionnaire to put their clinician's findings on record. It covers the diagnosis, medications and whether symptoms are refractory to therapy, the symptom list from widespread pain to irritable bowel and Raynaud's-like symptoms, how often symptoms occur, and tender points by site and side. The form puts particular weight on how often symptoms occur and whether they respond to therapy.

Who completes it

The Veteran's treating or examining clinician completes the form, including the dominant hand, and signs the Section IX examiner's certification. The Veteran fills in nothing but describes the history and symptoms.

Submitting and payment

How to submit it
The signed DBQ is returned to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may also file it.
Who pays
VA will not pay or reimburse the cost of completing the fibromyalgia DBQ, so any fee is agreed between the Veteran and the provider.

Good to know

  • For VA purposes widespread pain must affect both sides of the body, above and below the waist, and both the axial skeleton and the extremities.
  • Item 3B grades frequency as episodic with exacerbations, present more than one-third of the time, or constant or nearly constant, and notes triggers such as stress or overexertion.
  • Depression or another mental health condition due to fibromyalgia requires the separate Mental Disorders questionnaire as well.
  • The form states that imaging studies are not required to document fibromyalgia.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the VA fibromyalgia DBQ require tender point testing?

Item 3C asks whether tender points are present and, if so, which ones by side: low cervical, second rib, occiput, trapezius, supraspinatus, lateral epicondyle, gluteal, greater trochanter and medial knee. The examiner can check all bilaterally. No imaging is needed to document the diagnosis.

What does refractory to therapy mean on this form?

Item 2D asks whether the Veteran's fibromyalgia symptoms are refractory to therapy and, if so, to describe it. The form pairs this with whether continuous medication is required and what current treatment is under way, so record the medications tried and the response.
