VA Eating Disorders DBQ (Anorexia, Bulimia)

At three pages, this is one of VA's shortest public DBQs, used when a Veteran claims anorexia, bulimia or another specified feeding or eating disorder. The examiner records the diagnosis and who made it, the history, and which rating findings apply: binge eating with vomiting or other compensatory measures, weight loss below 80 or 85 percent of expected minimum, incapacitating weeks per year, or repeated hospitalization for tube feeding or parenteral nutrition.

Who completes it

A mental health examiner completes the form and signs the Section VII certification. The form's note says initial Mental Disorders examinations must be done by a board-certified or board-eligible psychiatrist or a licensed doctorate-level psychologist, or by certain trainees under close supervision.

Submitting and payment

How to submit it
The signed DBQ goes to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may also submit it.
Who pays
VA does not pay or reimburse any cost of completing the eating disorders DBQ; any fee is between the Veteran and the examiner.

Good to know

  • For VA purposes an incapacitating episode is a period that requires bed rest and treatment by a physician, and the form grades these by total weeks per year.
  • Review examinations may also be done by a licensed clinical social worker, NP, clinical nurse specialist or PA under close supervision of a psychiatrist or doctorate-level psychologist who co-signs.
  • Item 1A asks for the date of diagnosis and the name of the diagnosing facility or clinician for each of bulimia, anorexia, or other specified feeding or eating disorder.
  • The form instructs the examiner to stop the interview if the Veteran has a mental health emergency and get help through local resources or the Veterans Crisis Line.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete the VA eating disorders DBQ?

The form applies VA's Mental Disorders examiner rules. Initial exams require a board-certified or board-eligible psychiatrist or a licensed doctorate-level psychologist, or a resident or psychology trainee under close supervision. Close supervision means the supervising psychiatrist or psychologist meets the Veteran, confers on the diagnosis and co-signs the report.

What weight findings does the eating disorders DBQ ask about?

Section III lists self-induced weight loss to less than 80 percent and to less than 85 percent of expected minimum weight, and resistance to weight gain even when below expected minimum weight. The examiner checks whichever findings apply alongside binge behaviors and incapacitating episodes.
