VA Ear Conditions DBQ (Vestibular and Infectious)

Veterans claiming Meniere's syndrome, peripheral vestibular disorders, BPPV, chronic otitis externa or media, mastoiditis, cholesteatoma or ear neoplasms use this 9-page VA questionnaire. The clinician records how often vertigo, staggering and attacks occur and how long they last, findings of ear infection or inflammation, prior ear surgery, and an exam including the tympanic membrane, Romberg, Dix-Hallpike and finger-nose-finger tests.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XII examiner's certification. Hearing loss and tinnitus are handled on a separate Hearing Loss and Tinnitus questionnaire, not on this one.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the disability claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may also submit it.
Who pays
VA will not pay or reimburse any cost of completing the ear DBQ, so any fee is agreed between the Veteran and the provider.

Good to know

  • If the Veteran reports hearing loss or tinnitus from any listed ear condition, the Hearing Loss and Tinnitus Questionnaire must also be completed; for otosclerosis it is used instead of this form.
  • VA lists the Hearing Loss and Tinnitus DBQ among those not available for public use, citing federal regulations or specialized training needed to complete it.
  • Vestibular symptoms are graded by frequency (less than monthly, 1 to 4 times a month, more than weekly) and episode length (under 1 hour, 1 to 24 hours, over 24 hours).
  • Item 9B asks whether an audiogram has been done and to attach or provide the results; existing tests that reflect the current condition need not be repeated.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a private doctor complete the VA DBQ for Meniere's disease?

Yes. Meniere's syndrome is the first diagnosis listed on the Ear Conditions DBQ. Because Meniere's involves hearing impairment, the form says the Hearing Loss and Tinnitus Questionnaire must also be completed, and VA does not publish that questionnaire for private providers.

Which exam tests does the ear DBQ record?

Section VI covers the external ear and auricle deformity, the ear canal, the tympanic membrane including perforations, gait, the Romberg test, the Dix-Hallpike (Nylen-Barany) test for vertigo or nystagmus, and finger-nose-finger limb coordination. Each item can be marked not indicated if that test does not apply.
