VA Diabetic Peripheral Neuropathy DBQ

When a Veteran claims peripheral neuropathy from diabetes, this 9-page questionnaire documents each affected nerve separately. The clinician records pain, paresthesias and numbness by limb, a full neurologic exam, then grades the radial, median, ulnar, sciatic and femoral nerves on each side from normal to complete paralysis, plus assistive devices and any loss of effective limb function.

Who completes it

The Veteran's treating or examining clinician completes all sections and signs the Section 12 examiner's certification, including NPI and license number. The Veteran completes no part of the form.

Submitting and payment

How to submit it
The signed DBQ is given to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative can also submit it.
Who pays
VA does not pay or reimburse the cost of completing this DBQ; any fee is between the Veteran and the provider.

Good to know

  • The form states EMG is rarely required: the diagnosis can rest on stocking-glove symptoms plus objective findings such as reduced reflexes, strength, or cold, vibration, position or monofilament sensation.
  • For VA purposes, wholly sensory nerve impairment should be rated mild or, at most, moderate, and the form prints the picture of complete paralysis for each nerve.
  • The sensory exam covers light touch or monofilament by region, position sense at the index finger and great toe, vibration with a low-pitched tuning fork and cold sensation.
  • This DBQ is often completed alongside the Diabetes Mellitus DBQ, which lists diabetic peripheral neuropathy as a complication that requires this separate questionnaire.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is an EMG required for the VA diabetic neuropathy DBQ?

No. The form says EMG studies are rarely required, because diabetic peripheral neuropathy can be diagnosed in the right clinical setting from characteristic pain or sensory changes in a stocking-glove pattern and objective exam findings. If EMG has been done, item 9A records the limbs tested, results and dates.

Which nerves are graded on this DBQ?

In the upper extremities the radial, median and ulnar nerves; in the lower extremities the sciatic and femoral nerves. Each side is marked normal, incomplete paralysis or complete paralysis, and incomplete paralysis is graded by severity, with the sciatic nerve also allowing moderately severe and severe with marked muscular atrophy.
