VA Cranial Nerve Conditions DBQ (Disability Form)

Trigeminal neuralgia, facial nerve palsy and disorders of cranial nerves IX to XII are documented for VA disability claims on this 8-page questionnaire. The clinician maps pain, numbness and paresthesias by facial zone and side, rates chewing, swallowing and speech difficulty, tests motor strength and facial sensation, then grades each nerve as unaffected, incompletely or completely paralyzed. VA uses that grading when it assigns the rating.

Who completes it

The Veteran's treating or examining clinician completes every section and signs the Section XI examiner's certification with NPI, license and specialty. No part is completed by the Veteran.

Submitting and payment

How to submit it
The clinician returns the signed form to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it on the Veteran's behalf.
Who pays
VA does not pay or reimburse any cost of completing this cranial nerve DBQ, so any charge is settled between the Veteran and the provider.

Good to know

  • Cranial nerves I, II to IV, VI and VIII are rated on other questionnaires: Loss of Smell and Taste, Eye Conditions, and Hearing Loss and Tinnitus, completed in addition to or instead of this one.
  • Section VI grades each nerve from not affected to complete paralysis; for VA purposes incomplete paralysis means function lost substantially less than the complete paralysis described for that nerve.
  • The form notes that imaging or other diagnostic studies are usually not required to diagnose cranial nerve conditions in the appropriate clinical setting.
  • Functional impact in Section IX must describe only the diagnosed cranial nerve conditions, without considering other medical conditions or factors such as age.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does trigeminal neuralgia go on the VA Cranial Nerves DBQ?

Yes. Cranial nerve V (trigeminal) is one of the nerves this form evaluates. The examiner records constant, intermittent or dull pain, paresthesias and numbness for the upper, mid and lower face on each side, tests jaw-clench strength and facial light-touch sensation, and then grades the degree of paralysis.

Is the Hearing Loss and Tinnitus DBQ available to private providers?

No. The cranial nerves form sends cranial nerve VIII problems to the Hearing Loss and Tinnitus questionnaire, but VA lists that DBQ among those not available for public use, explaining that federal regulations or specialized training limit who can complete it.
