APS Medical Care Program Application (Arizona)

Arizona Public Service uses this bilingual application to enroll households in its Medical Care Program, which sends advance notice of planned outages. A household qualifies when a member depends on life-sustaining medical equipment that would be especially dangerous to lose for more than four hours, or has a condition that substantially limits self-care or self-protection. The application must be renewed every year and after any move.

Who completes it

The customer completes section 1 (account, service address, phones, optional extra contact). A licensed medical professional, meaning an allopathic or osteopathic physician, registered nurse practitioner or physician assistant, completes section 2: patient name, eligibility met or not met, printed name, phone, date, signature, and state and license number.

Submitting and payment

How to submit it
The completed application is returned to the APS Medical Care Program by mail, the only channel the program lists.
Who pays
The application lists no fee for the medical professional's verification.

Good to know

  • By signing, the professional confirms the patient lives at the listed address, qualifies, and has discussed contingency plans with them for an interruption of electric service.
  • It is a notification program only: it is not a discount and does not guarantee uninterrupted service, and customers must stay current on their bills.
  • The bill discount program, Energy Support with Medical, needs its own separate application.
  • All section 1 fields are required, in blue or black ink. After a move, alerts do not apply at the new address until a new certification is approved.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the APS Medical Care Program application?

A licensed medical professional: an allopathic or osteopathic physician, a registered nurse practitioner, or a physician assistant. Section 2 asks for the professional's state and license number, phone number, signature and date, and a circled choice of eligibility criteria met or not met.

Does the APS Medical Care Program stop disconnection?

No. It notifies the household about planned outages, their status and the estimated restoration time. APS states it is not a discount program and does not guarantee uninterrupted service, and customers must keep paying their monthly bills to remain enrolled.

Is the APS application available in Spanish?

Yes. The PDF is bilingual, with the English application on page 1 and the Spanish Solicitud del Programa de Cuidado Médico on page 2. Both versions have the same customer section and licensed medical professional verification.
