Chicago Housing Authority Certification of Need

Chicago Housing Authority public housing and voucher households request disability accommodations, meaning changes to CHA policies, procedures or properties, through CHA's online Reasonable Accommodation Portal. A knowledgeable professional who can verify the disability then completes a certification of need for each requested accommodation in the same portal workflow. CHA decides within about 30 days.

Who completes it

The household member submits the request after verifying their account details. A knowledgeable professional, such as a physician or other licensed provider, certifies the disability and the disability-related need; when several professionals are needed, each completes their own certification.

Submitting and payment

How to submit it
Everything goes through CHA's Reasonable Accommodation Portal; CHA does not publish a downloadable certification form.
Who pays
CHA's portal lists no fee or payment for the professional's certification.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a paper CHA certification of need form?

CHA does not publish one. Requests and certifications are handled in CHA's online Reasonable Accommodation Portal: the household member submits the request, and the knowledgeable professional completes the certification of need for the disability and each requested accommodation through the portal workflow.

What if more than one professional must verify the disability?

The portal states that when a household requests several accommodations and needs more than one professional to verify the disabilities, each knowledgeable professional must complete their own certification of need. One professional cannot certify on behalf of another.
