Utah Form 123: Physician's Initial Report of Injury

Utah requires the treating physician to file this first report for work injuries or occupational diseases that involve lost time or medical treatment. It captures the employee's own account of how the injury happened, the diagnosis with an ICD code, whether the condition results from the described work injury or exposure, and whether the employee needs an interpreter. Rule R612-100-3(H) of the Utah Administrative Code requires it.

Who completes it

The treating physician completes the whole form, including the employee's statement of cause written in the first person. The employer and carrier file their own first reports on Forms 122E and 122C.

Submitting and payment

How to submit it
Within 7 days of the initial visit, send a copy with the bill and progress notes to the insurance carrier and form-only copies to the employee, employer and Labor Commission, or file online through Easy Online 123.
Who pays
Form 123 sets no fee of its own; the carrier's copy is sent together with the provider's bill and progress reports.

Good to know

  • Work-relatedness is answered yes, no or undetermined, so a physician can file on time before causation is settled.
  • The interpreter question asks whether the claimant needs one and in what language.
  • Filing online through the Commission's Easy Online 123 system requires registration first.
  • The current version is Rev 5/2025 and includes a registered email field alongside the treatment facility and physician contact details.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is Utah Form 123 due?

Within 7 days of the initial visit. The physician sends the carrier a copy with the bill and progress reports, and sends copies of the form alone to the injured employee, the employer and the Utah Labor Commission, which accepts it by fax or online through Easy Online 123.

Do I file Form 123 if causation is unclear?

Yes. Item 20 asks whether the condition requiring treatment results from the industrial injury or exposure described and offers yes, no or undetermined. The report is required for claims with lost time or medical treatment, so it is filed even while the work relationship is still being sorted out.
