Utah Form 043: Attending Physician's Statement

Injured workers in Utah who plan to leave the area or the state, or change doctors or hospitals, file Form 044 with the Labor Commission, and this statement must go with it. The worker sends it to the last physician who treated them in Utah. That physician reports the worker's condition at the last exam and, if the worker is not released to work, the expected stabilization date, further treatment and likely impairment.

Who completes it

The worker completes the top section with name, mailing address, date of injury, employer and carrier. The last attending physician in Utah completes items 1 to 3 and signs.

Submitting and payment

How to submit it
The physician returns the completed form to the injured worker as soon as possible, and the worker sends it with Form 044 to the Labor Commission's Division of Industrial Accidents.
Who pays
Form 043 sets no fee and does not say who pays for completing it.

Good to know

  • Item 1 records the employee's condition when last examined and the date of that last examination by the Utah physician.
  • If the worker was not released to work, item 2 asks for an estimated date of stabilization or return to work, additional treatment needed, and the probability and extent of permanent partial impairment.
  • If the physician is referring the worker to another doctor, clinic or hospital, item 3 asks which one and a brief explanation of the referral.
  • The current form is Rev 10/2019 and is a fillable PDF on the Commission's industrial forms page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is Utah Form 043 needed?

It must accompany Form 044, the Employee's Notification of Intent to Leave Locality or State and to Change Doctor or Hospital. The injured worker asks the last Utah physician who treated them to complete it, then files both forms with the Labor Commission's Division of Industrial Accidents.

Who sends Form 043 to the Labor Commission?

The injured worker. The form tells the physician to complete it and send it back to the worker as soon as possible, and the worker then mails it with Form 044 to the Division of Industrial Accidents. The physician does not file it directly with the Commission.
