Tennessee LB-0931A: MIR Report, AMA Guides 6th Edition

Tennessee Medical Impairment Rating (MIR) Registry physicians use this Word-format report for injuries on or after January 1, 2008, when the impairment rating is the only issue in dispute. It walks through clinical evaluation, analysis of the findings and a diagnosis-by-diagnosis rating under the AMA Guides, 6th edition with its errata. The final whole person impairment entered on page one is legally presumed to be the correct rating.

Who completes it

The MIR Registry physician keys in every response and signs the certification. Examinee questionnaires such as the QuickDASH or the Pain Disability Questionnaire are completed by the injured worker when used.

Submitting and payment

How to submit it
Send the completed report with required attachments and a curriculum vitae to the Bureau's MIR program by secure email or fax, as the form directs, before the due date.
Who pays
The employer bears the cost of the MIR evaluation, whichever party requested it.

Good to know

  • If the examinee is not at MMI, explain why and what is needed, and rate the impairment anyway; additional treatment requires documentation that the patient wants it.
  • Each ratable diagnosis needs its chapter, table, class, grade modifiers for functional history, physical exam and clinical studies, and the net adjustment calculation.
  • The mental and behavioral section is completed only by a psychiatrist, using the median of the BPRS, GAF and PIRS impairment scores.
  • If a prior work-related impairment rating exists, calculate the final rating both with and without subtracting it.

Frequently asked questions

Which Tennessee MIR form applies to a 2015 injury?

The 6th edition report, LB-0931A. The Bureau states that injuries on or after January 1, 2008 must be rated under the AMA Guides to the Evaluation of Permanent Impairment, 6th edition. The 5th edition form, LB-0931, is for injuries before that date.

Must the final rating be written out in words on LB-0931A?

Yes. Page one asks for the final whole person impairment both in numbers and in words, directly above the physician's certification, and notes that this is the final rating legally presumed to be the correct impairment rating.
