Tennessee C-32: Standard Form Medical Report

Tennessee's Standard Form Medical Report lets a treating or evaluating physician give direct testimony in writing instead of sitting for a deposition. The three-page form covers the course of treatment, time off work and restrictions, permanent impairment, and detailed causation questions. A party files it with the Court of Workers' Compensation Claims, and an opposing party may object and demand a deposition instead.

Who completes it

The treating or evaluating physician completes the whole form, checks which role applies, and signs the certification with an original signature. No other party completes a section.

Submitting and payment

How to submit it
Return it to the requesting party, who files it with the Court of Workers' Compensation Claims and serves the other parties.
Who pays
The C-32 sets no fee and does not say who pays for preparing it.

Good to know

  • For injuries on or after July 1, 2014, causation answers must reflect a reasonable degree of medical certainty, and 'more likely than not' and 'primarily' both mean greater than fifty percent.
  • Attach a Final Medical Report (Form C-30A) to the impairment section; if the AMA Guides do not adequately assess impairment, give your own percentage with a detailed explanation.
  • The physician certifies that a curriculum vitae is attached and accurate, so the CV must go with the signed form.
  • Separate questions cover aggravation of a pre-existing condition, asking whether employment was primarily responsible for worsening it or for the need for treatment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a C-32 replace a physician's deposition in Tennessee?

Yes. The form says it may be used to introduce direct testimony in lieu of a physician's deposition under Tenn. Code Ann. 50-6-235(c), and it should bear the physician's original signature. An opposing party can object and demand a deposition, so the report must stand on its own as testimony.

What causation standard does the C-32 use?

For injuries on or after July 1, 2014, the injury is medically caused by employment if the employment activity, more likely than not, is primarily responsible for the injury or the need for treatment, both meaning greater than fifty percent. For earlier injuries, the form asks whether the injury more probably than not arose out of employment.
