Connecticut W-1461 Employment Services Medical Report

When a Connecticut DSS client says a medical condition keeps them from the department's employment programs, DSS mails this report to the treating provider. The provider states whether the condition prevents work, the written diagnosis, how long the inability will last, the return-to-work date and any part-time capacity, plus history, symptoms and prognosis, so DSS can decide on a temporary exemption.

Who completes it

A licensed provider currently treating the patient, whose scope of practice permits diagnosing and treating the condition, completes everything below the DSS worker's header and signs with license number. A licensed master social worker may complete it for mental health disorders with a qualified co-signer.

Submitting and payment

How to submit it
Return it directly to the DSS Scanning Center using the envelope and cover sheet DSS supplies, with the address showing in the envelope window.
Who pays
The form names no fee; for a patient with no medical insurance and no DSS medical assistance, the provider must also complete the W-513 form included in the packet.

Good to know

  • Enter a written diagnosis only; the form asks providers not to use ICD-9 or ICD-10 codes.
  • If the answer to question 1 is no, or the inability to work will last less than 30 days, stop and go straight to the signature on page 3.
  • An LMSW's report on a mental health disorder needs a co-signature from a supervising physician, APRN, psychologist, professional counselor or LCSW.
  • DSS will not accept the form without the provider's signature; the signature block also asks for provider type and license number.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can sign Connecticut's W-1461?

A licensed medical provider whose scope of practice under the Connecticut General Statutes permits them to diagnose and treat the condition, such as an MD, DO, APRN, PA or psychologist. A licensed master social worker may complete it for mental health disorders with a supervising clinician's co-signature.

Can I use ICD-10 codes on the W-1461?

No. The form asks for a written diagnosis only and tells providers not to use ICD-9 or ICD-10 codes. Describe the incapacitating symptoms or impairments and how they prevent work in the medical information section instead.

What if my patient cannot work for less than 30 days?

Check 'Less than 30 days' and enter the onset and return-to-work dates, then stop and go to the signature instructions on page 3. The part-time capacity and medical information questions are answered only when the inability to work is expected to last 30 days or more.
