Texas H1836-B Medical Release or Physician's Statement

Texas Works advisors use this form when a TANF or SNAP recipient says they cannot work because they must care for a disabled family member, or when TANF hardship exemptions are requested for that reason. The patient's physician states how much the caregiver can work or prepare for work and gives the family member's disabling diagnoses.

Who completes it

The patient's medical provider completes Section 2 (Part A caregiver work capacity and Part B diagnosis) and signs with license number. HHSC staff complete Section 1, and the patient or a personal representative signs the Section 3 release.

Submitting and payment

How to submit it
After completing it, give the form to the caregiver or mail it to the HHSC office in Section 1; the instructions also allow faxing it to the advisor.

Good to know

  • Part A choices: can work full time; part time outside or inside the home (with hours per week); or cannot work, with the disability marked permanent, over 6 months, or 6 months or less.
  • Advisors also use it for TANF hardship exemptions during the state time-limit freeze-out period and for extended hardship exemptions at or after the 60th month of assistance.
  • The patient's Section 3 authorization lets HHSC and the Texas Workforce Commission receive the information; it is usually set to expire when benefits expire.
  • The current edition is June 2025; a Spanish version, H1836-BS, is posted. The worker files a copy in the medical section of the case record.
