Texas HHSC Form 3055 Physician's Orders (DAHS)

Texas Day Activity and Health Services (DAHS) providers send this form to a person's physician to obtain orders for initial DAHS approval, new orders after a change in condition, or supplemental orders for nursing services. The physician certifies that the person has a chronic medical condition requiring care, monitoring or intervention by a licensed nurse.

Who completes it

The DAHS provider completes Section I (the person's information and DAHS nurse) and sends a copy to the physician, who completes the rest: diagnoses with ICD-10 codes, functional limitations, diet, medications, treatments and the Section VI certification with signature, license and individual NPI.

Submitting and payment

How to submit it
Return the signed orders to the DAHS provider, which keeps the original and sends a copy with Forms 3050 and 2101 to the HHSC regional nurse or managed care organization.

Good to know

  • List current, pertinent chronic diagnoses from the last 24 months with ICD-10 codes; symptoms should not be listed as diagnoses.
  • The primary diagnosis cannot be an intellectual or developmental disability or a mental health condition.
  • The physician also certifies they are not an owner, partner or member of the requesting DAHS provider.
  • Use the individual NPI, not a group NPI; licenses from Texas or a contiguous state (Arkansas, Louisiana, Oklahoma, New Mexico) are accepted.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What does a physician certify on Texas Form 3055?

That the person has a chronic medical condition requiring care, monitoring or intervention by a licensed nurse, that the primary diagnosis is not an intellectual or developmental disability or mental health condition, and that the physician is not an owner, partner or member of the requesting DAHS provider.

Which medications go on Form 3055?

All medications given at home or by the DAHS provider, including as-needed over-the-counter drugs, with dosage, route, frequency and related diagnosis. The instructions include medications the person takes independently or with reminders, and ordered therapies or treatments with frequency.

Can I certify no significant change on Form 3055?

Yes. Checking the box and dating it in Section VI certifies there is no significant change from the previous assessment. In that case the DAHS provider must also send the previous assessment when requesting authorization.
