NH DCYF Form 1722 Medical Information Statement

New Hampshire's Division for Children, Youth and Families uses this two-sided statement to verify the health of foster and adoptive family applicants and the people in their household. The individual lists their illness history and authorizes release; the provider then records significant health history, immunizations and an impression of whether the person is free of impairments that could affect children's care.

Who completes it

Only a currently licensed physician, physician assistant or APRN may complete and sign the provider side, checking their profession. The individual or legal representative completes the illness history and signs the release on the front, which lasts two years.

Submitting and payment

How to submit it
The provider faxes or mails the completed statement to the DCYF Foster Care Program, as printed at the bottom of the provider side.
Who pays
The form states that payment of any separate charge for completing it is the patient's responsibility.

Good to know

  • For all adult household members and caregivers, record pertussis (whooping cough) and annual influenza immunization dates, or the date of illness.
  • For children, a copy of the current immunization record must be attached, and you indicate whether the child is up to date per AAP, ACIP and AAFP recommendations.
  • Significant health history covers chronic illnesses, hospitalizations and regular medications, alongside the date of the last physical exam.
  • The impression section asks whether the person was found free or not found free of physical and mental impairment that may adversely affect children's care.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
