NH BFA Form 720 FANF Determination of Incapacity

New Hampshire's Financial Assistance to Needy Families (FANF) program can qualify a parent whose physical or mental incapacity substantially reduces their ability to support or care for their children. This two-sided form pairs the applicant's health information release with a provider statement certifying the incapacity, its start and expected end, the diagnosis and its basis, and current and recommended treatment.

Who completes it

A currently licensed provider from the professions listed on the back completes and signs the provider side. The applicant completes the front, a release that can include specially protected mental health, substance use disorder and HIV information if initialed.

Submitting and payment

How to submit it
Mail the completed form to the DHHS Centralized Scanning Unit, as printed at the bottom of the provider side, for routing to the Family Services Specialist.
Who pays
The form states that payment of any separate charge for completing it is the responsibility of the patient.

Good to know

  • The incapacity must be expected to last 30 continuous days from the FANF application date, or must have lasted 30 continuous days in the 90 days before it.
  • Eligible signers include physicians, PAs, APRNs, board-certified psychologists, clinical mental health counselors, pastoral psychotherapists, independent clinical social workers, MLADC counselors and marriage and family therapists.
  • Mark whether the diagnosis is based on an examination, medical records or another source, with dates; the childcare box flags a parent unable to supervise their children.
  • The applicant's authorization ends one year from signing and can be revoked in writing; the edition is BFA Form 720, 06/19.
