Maryland TDAP Medical Report Form 500-C

Maryland's Temporary Disability Assistance Program (TDAP) is state-funded cash help for adults whose impairment keeps them from working, short term or while an SSI claim is pending. Form 500-C is the medical evidence for it. The provider gives the clinical diagnosis, substance use status, whether the impairment prevents work or training, and whether it will last at least 3 months or 12 months or more.

Who completes it

A health care provider with independent diagnostic authority, such as a physician, NP, PA, licensed psychologist or LCSW-C, completes Sections B to E and signs with title and license number. The local Department of Social Services fills in the office, case manager and customer details.

  • If substance abuse is present, the form asks whether other medical conditions exist in addition to it.
  • Section C asks for the clinical diagnosis and the name of the impairment, and Section D whether it is severe enough to prevent work or training.
  • A Spanish edition revised 4/2019 is posted; the general FIA medical report is Form 500.
