Maryland DHS/FIA 500-A Verification of Disability

Maryland SNAP recipients subject to the work requirement for able-bodied adults can claim they cannot work because of a disability, pregnancy or caring for an ill household member. This one-page form, revised in December 2025 after the federal SNAP work-rule changes, records that claim, and when the caseworker asks, a health care provider verifies the period the person is unable to work.

Who completes it

Any certified and licensed health care professional providing care to the person completes and signs Section 2, only when the caseworker requests it. The customer completes and signs Section 1.

Submitting and payment

How to submit it
The completed form goes back to the local Department of Social Services handling the SNAP case.
Who pays
The form sets no fee and does not say who pays for completing Section 2.

Good to know

  • Section 2 must give both a begin and an end date; the form says not to write forever, indefinite or unknown.
  • A physician's signature is not required: nurse practitioners, physician assistants, psychologists, licensed social workers and therapists can complete Section 2.
  • Section 2 is needed only if the caseworker requests it; FIA's SNAP manual says an obvious disability needs no verification.
  • A fillable Spanish edition of the December 2025 version is posted alongside the English form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a doctor have to sign Maryland's 500-A?

No. The form states it may be signed by any certified and licensed health care professional providing care to the person, and FIA's SNAP manual confirms a physician's signature is not required. The provider gives their license number and state, medical group and contact details.

What end date should go on the 500-A?

A specific date. Section 2 asks for the begin and end dates of the period the person is unable to work or participate in a work activity, and tells providers not to use forever, indefinite or unknown. The signature verifies the person is unable to work for that period due to a disability.
