Alabama DHR-2092 Medical Report for Child Placing Staff

Alabama child placing agencies and residential child care facilities need a medical report showing that foster or adoptive parents and staff can safely care for children. On this one-page form the examiner records a Mantoux TB test, medications that affect alertness or driving, physical limitations and observed psychological or emotional concerns, then states the person is free of infectious disease and physically fit.

Who completes it

A medical doctor, physician assistant or certified nurse practitioner completes everything below the applicant's authorization and signs. The applicant fills in their name, agency or facility and position and signs the authorization to release the report.

Submitting and payment

How to submit it
The report goes to the child placing agency or residential facility named on the form for its licensing file.
Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • An initial Mantoux TB test is required, with a repeat every two years unless extenuating circumstances exist; a positive result needs a chest X-ray date and result.
  • The examiner answers yes or no on medications affecting alertness, reactions or driving, physical limitations, and psychological or emotional behaviors that limit child care.
  • Any referral to another health professional is described in the Comments section of the form.
  • Children in the same placements use the separate DHR-FCS-623 Child's Medical Record. The adult report is dated Revised June 2006.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
