Florida CF-FSP 5073 Physician's Report on Child

On this one-page DCF form the doctor marks each body system normal or describes abnormalities, notes other significant findings and recommendations, past illnesses and surgery, and evaluates physical and mental development. A syphilis test, or the mother's test at delivery, is recorded, with other labs if indicated.

Who completes it

The examining physician completes and signs the whole form, including how long the child has been under their care and how often they have seen the child.

Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • A blood test for syphilis is the only lab listed without an 'if indicated' qualifier, and the mother's test at delivery can be recorded instead.
  • Urine, blood count, vaginal smear, tuberculin and X-ray results are entered only if indicated, each with a date and result.
  • The child's family, social and medical background is recorded separately on CF-FSP 5074, Family, Social and Medical Information About Child to Be Adopted.
  • Release of health information to prospective foster or adoptive parents uses CF-FSP 5230. The physician's report is dated PDF 10/2005.
