Florida CF-AA 1020A Home Care for Disabled Adults

Florida's Home Care for Disabled Adults program pays a subsidy to relatives or others who give a disabled adult 24-hour care at home instead of a nursing home. DCF Adult Services sends this form to the applicant's physician or registered nurse to verify that the medical condition could warrant nursing home placement without supportive in-home services.

Who completes it

A physician or registered nurse completes Sections I to IV and signs. DCF Adult Services fills in the addressee and applicant details and attaches the applicant's signed Confidential Information Release.

Good to know

  • Section I lists each disease, whether it is stabilized or controlled, and its medications or treatments.
  • Physical and mental limitations in Section II each need a stated duration.
  • Section IV is a single choice: the program is appropriate because the 24-hour caregiver is an alternative to institutional care, or it is not appropriate.
  • The subsidy covers basic support and maintenance, basic medical costs and a special supplement for health-related or in-home supportive services, as the form explains.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete Florida's HCDA medical statement?

A physician or registered nurse. DCF addresses the form to them, attaches the applicant's signed Confidential Information Release, and asks them to list diseases and treatments, physical and mental limitations with duration, a statement of general health and a recommendation on living arrangements.

What does 'not appropriate' mean on the CF-AA 1020A?

The 'not appropriate' choice applies when the applicant does not need a 24-hour caregiver and can live independently at home, or when an institutional setting is more appropriate. The other choice confirms the program fits because the 24-hour caregiver offers an alternative to nursing home or other institutional care.
