Nebraska CC-6 Special Needs Child Care Documentation

Nebraska DHHS uses this form when a family or provider requests a higher child care rate because of a child's special needs. The health care professional checks the qualifying conditions, gives the diagnosis, explains why the condition needs special child care accommodations or attention, and estimates how long it will last. Approved special needs rates last up to six months.

Who completes it

A physician, licensed or certified psychologist, or licensed mental health practitioner completes page 1 from item 1 onward and signs with printed name and specialty. The parent supplies the child's details; page 2 is completed by the DHHS resource development worker.

Submitting and payment

How to submit it
The parent has the child's health care professional complete, sign and return the form, and it goes back to DHHS, where page 1 is used by the social service worker.

Good to know

  • Qualifying conditions must not be related to chronological age, and must be documented by a physician, licensed or certified psychologist, or licensed mental health practitioner.
  • The checklist covers emotional, developmental, movement, sensory and speech impairments, plus help with hygiene, feeding, toileting, medical procedures, therapy and medications.
  • Special needs rate exceptions run for up to six months, and a new evaluation is required if the child's needs continue.
  • Page 2 records what the child care provider needs, such as extra staffing, special supplies, equipment or environmental changes; the clinician does not complete it.
