Nebraska CC-3E Incapacity Statement (Child Care)

Nebraska DHHS asks for this one-page statement when a parent or caretaker requests child care services because they cannot care for their own children. The physician confirms whether the client meets the DHHS definition of incapacity, then describes its nature, expected duration, specific restrictions, the child care tasks the client cannot do and the weekly hours affected.

Who completes it

The examining physician (the signature line specifies Medical Doctor) completes item 3 onward, including the incapacity finding, and signs with printed name and address. The DHHS service worker fills in the client, worker and local unit details in item 1.

Submitting and payment

How to submit it
Return the statement to the DHHS service worker and local unit listed in item 1; the worker places it in the client's case file and no extra copies are needed.

Good to know

  • DHHS defines incapacity as a physical or mental illness, impairment or defect so severe that it eliminates the caretaker's ability to provide care for their children.
  • If the duration is unknown, give an estimated date when the condition will be reevaluated rather than leaving the field blank.
  • Examples of activities to list include being unable to supervise, diaper, dress or bathe the children, or prepare meals; examples must fit the children's ages or special needs.
  • The form is used only when child care is requested because of incapacity; Form cc-28 covers child care while a parent attends medical appointments.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
