California ADM 36 Medical Report, Child to Be Adopted

California adoption investigations need a physician's report on the child. The ADM 36 pairs the caretakers' account of birth, nutrition, development and illnesses with the physician's exam, lab results such as blood serology, toxicology and newborn screen, immunization status, risk factors, signs of abuse or neglect, and a diagnosis and recommendation. Independent infant adoptions need a second report later.

Who completes it

The physician who examined the child completes Section B and signs. Section A is filled out by the caretakers or adopting parents before the exam, including which report this is.

Submitting and payment

How to submit it
Return the completed form to the adoption agency or CDSS office written in the 'Return to' box at the top.
Who pays
The form sets no fee and does not say who pays for the examination.

Good to know

  • A second medical report is required for independent infant adoptions once the child is at least 5 months old; agency adoptions use a single (sole) report.
  • Blood serology, toxicology screen and PKU/newborn screen each need a date and result or a check in the 'medically not indicated' box.
  • The physician states how many times they have seen the child and whether the child appears to be parented in a way that meets medical and developmental needs.
  • The related AD 512, Psychosocial and Medical History of Child, records the child's background separately.
