California CW 61B Mental Capacities (CalWORKs)

The CW 61B is the mental health page of California's CW 61 medical packet for CalWORKs participants. Instead of checkboxes, the provider writes how the person's current mental condition affects four areas: daily activities, social functioning, task completion and adaptation to work-like stress. The county uses it to match the participant to a work, training or education activity they can manage.

Who completes it

The licensed physician, psychiatrist or psychologist completing the CW 61 writes the four narrative answers and signs as provider or evaluator (a designee may sign). The county fills in the case details and describes the assigned CalWORKs activity and hours.

Submitting and payment

How to submit it
Return it to the county welfare department with the CW 61, attaching additional documentation if the space on the page is not enough.
Who pays
The CW 61B sets no fee and does not say who pays for completing it.

Good to know

  • Task completion covers following simple written or oral instructions, sustaining focused attention and finishing everyday workplace, training or school routines.
  • Adaptation asks about coping with common stresses such as decision making, attendance, schedules and dealing with supervisors or instructors.
  • Answers should relate to the specific CalWORKs activity the county lists at the top of the page, when one has been assigned.
  • Like the CW 61A, it is dated 6/01 and marked 'required form, substitute permitted'.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
