OPM RI 30-10: Disabled Dependent Questionnaire

OPM uses RI 30-10 to decide whether the adult child of a federal annuitant or deceased employee is incapable of self-support because of a disability that began before age 18, which affects survivor annuity and FEHB family coverage. The physician completes Part B on the back and attaches a narrative on letterhead covering eight points about the condition.

Who completes it

The disabled dependent, a guardian or another fiduciary completes Part A: living situation, any court appointment, employment and earnings in the last 12 months, and education. The dependent's physician completes and signs Part B.

Submitting and payment

How to submit it
The completed form and the physician's narrative are returned to OPM's Retirement Benefits Branch at the address printed on the form.

Good to know

  • Part B asks for the diagnosis, expected recovery date, age at onset, severity (mild, moderate or severe) and, for mental disability, approximate mental age and IQ test results.
  • The narrative covers history, recent clinical findings, current status and plans, whether the condition is static, limitations, ability to work, age self-support ended, and ability to handle finances.
  • If the dependent is married, Part A asks for a copy of the marriage certificate. Previous editions remain usable; the current revision is May 2018.
  • OMB number 3206-0179. Providing the information is voluntary, but missing information may prevent OPM from allowing benefits.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the physician need to attach to OPM RI 30-10?

A narrative on letterhead addressing eight points: history of the condition, findings from the latest evaluation including tests, current clinical status and treatment plans, whether the condition is static, physical or mental limitations, whether it precludes self-supporting work, the age at which the patient became incapable of self-support, and whether they can handle finances.

Who completes Part A of the Disabled Dependent Questionnaire?

The disabled dependent, or their guardian or other fiduciary. Part A covers the dependent's Social Security number, where and with whom they live, any court-appointed guardian, employment and earnings in the past 12 months, and highest education, followed by a certification that authorizes release of medical evidence to OPM.

What benefits does RI 30-10 affect?

OPM uses the questionnaire to determine whether an adult child of a federal annuitant or deceased employee remains eligible as a disabled dependent, which bears on a survivor annuity and on family coverage under the Federal Employees Health Benefits program. OPM's Privacy Act statement says the form determines eligibility for continued benefits.
