DD Form 2828: Physician Certificate for Child Annuitant

DFAS uses DD Form 2828 when a military retiree's adult child may qualify for a Survivor Benefit Plan or RSFPP child annuity because a mental or physical incapacity prevents self-support. A physician or psychiatrist who has attended the child certifies the diagnosis, onset date and whether the child is incapable of self-support or of handling their finances. The form goes to DFAS U.S. Military Annuitant Pay.

Who completes it

A licensed physician or psychiatrist who has attended the child completes items 5 through 8: diagnosis, date of onset, the physician's statement, and the certification with signature. The items before item 5 are not part of the physician's section.

Submitting and payment

How to submit it
The completed certificate is mailed to DFAS U.S. Military Annuitant Pay in Indianapolis.
