TRSL Form 12D: Physician Annual Report for Retirees

Teachers' Retirement System of Louisiana disability retirees return this two-page report to show that their disabling condition continues. The retiree fills in their details and forwards it to a physician, who records history, review of systems, social history, vitals, physical and neuropsychiatric exams, diagnosis with supporting test results, treatment, prognosis, and three opinions about returning to full-time work.

Who completes it

The retiree completes Section 1, retiree information, and forwards the form to the physician. The physician completes Section 2, the report of the patient's disabling condition, and a licensed physician signs Section 3.

Submitting and payment

How to submit it
Return the report to TRSL by drop-off, mail, email or fax. All entries except signatures must be printed in ink or typed.

Good to know

  • Form 12D is for retirees only; new applicants use Form 12C, the Physician Report of Disabling Condition, with their disability retirement application.
  • Objective clinical findings and laboratory evidence of the disabling condition must be of sufficient magnitude to justify the claim, and a dictated report must cover the same outline as the form.
  • Question 10C offers three answers: yes but not at this time, no because of a total and permanent disability, or cannot be determined at this time.
  • Section 3 must be signed by a licensed physician, with license number, area of specialty and date signed. The current revision is 07/21.

Physician Annual Report of Disabling Condition

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does question 10 on TRSL Form 12D ask?

It asks three opinions: whether the patient can return now to full-time work without restrictions in the same area of work as before, whether the condition is terminal or recovery highly improbable, and whether the patient will ever be able to return to such work.

Who must sign TRSL Form 12D?

Section 3 must be signed by a licensed physician, who also enters their name, license number, area of specialty, date signed and daytime telephone. The retiree completes Section 1 and is responsible for forwarding the form to the physician for Sections 2 and 3.

Can the physician send a dictated report instead of Form 12D?

Yes. Form 12D says that if the medical report will be dictated, it should include the information outlined on the form: history, review of systems, social history, vital signs, physical and neuropsychiatric examination, diagnosis with test results, treatment and response, and prognosis.
