TRSL Form 12C: Physician Report of Disabling Condition

Louisiana teachers applying for disability retirement through TRSL give this report to their physician after completing the applicant sections. The physician documents history, review of systems, exam, diagnosis with DSM or ICD codes, treatment, prognosis and functional abilities, with objective findings strong enough to show the member cannot keep performing current job duties. TRSL also uses it for a disabled child's survivor benefit.

Who completes it

The applicant completes Section 1, applicant information, and signs Section 2, the authorization to release medical records. The physician or health care provider completes and signs Section 3, the physician's report of disabling condition.

Submitting and payment

How to submit it
Submit the report to TRSL by drop-off, mail, email or fax, with copies of all medical records relating to the disabling condition, including laboratory and other diagnostic test results.

Good to know

  • TRSL requires all medical records related to the disabling condition with the report; incomplete or inadequate information may delay processing or lead to denial of the claim.
  • A dictated report is acceptable if it covers the same outline: history, social history, review of systems, vital signs, physical and neuropsychiatric exam, diagnosis, treatment and prognosis.
  • Psychiatric claims add places and dates of psychiatric hospitalizations and mental status when last seen, and the physician marks ABPN certified, ABPN qualified or neither.
  • Survivor claims for a disabled child pair Form 12C with Form 12PDMC; the member's application is Form 12, with Form 12A from the supervisor. Current revision 01/21.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a physician dictate a report instead of filling out TRSL Form 12C?

Yes. Form 12C says a physician who chooses to dictate the medical report should include the information outlined on the form, from history of present conditions through diagnosis, treatment and prognosis. Copies of all medical records must still accompany the report, and the physician signs Section 3.

What standard does TRSL apply to Form 12C?

The form asks for objective clinical findings and laboratory evidence of sufficient magnitude to justify the patient's claim that they can no longer perform current job-related duties. Diagnosis entries should include the results of the tests that document each diagnosis.

Is Form 12C used for anything besides teacher disability retirement?

Yes. The same physician report supports a survivor benefit claim for a member's disabled child, filed with Form 12PDMC, the Statement of Child's Permanent Disabling Condition. In that case the applicant section identifies both the applicant and the TRSL member.
