Wisconsin ETF ET-5909: Recertification Medical Report

Wisconsin Retirement System disability annuitants may be required to show each year that they remain disabled, and this is the physician's report ETF uses for that recertification. ETF sends it to the annuitant when recertification is due, and the physician confirms continued disability based on an examination within 12 months of signing.

Who completes it

The annuitant's physician completes the physician portion and signs it. ETF sends the form to the annuitant, who is responsible for getting it completed and back to ETF on time.

Submitting and payment

How to submit it
ETF sends the form when annual recertification is required, and it must be returned to ETF by the due date ETF sets.

Good to know

  • The last examination date must be within 12 months of the date the physician signs the Recertification Medical Report.
  • Missing the recertification due date causes the monthly disability benefit to be suspended and potentially terminated.
  • If the physician indicates the annuitant can work in any occupation paying more than the annual earnings limit, the disability benefit is terminated and the WRS account is re-established.
  • Recertification can continue annually up to normal retirement age, and annuitants also report earnings on ET-5905, the Annual Earnings for Disability Annuitants statement.

Frequently asked questions

How often is the Wisconsin ET-5909 required?

ETF may require annual medical evidence that an approved disability annuitant remains disabled. Annual recertification can continue as long as the annuitant receives monthly disability payments or up to normal retirement age, and ETF sends the form when it is due.

What happens if ET-5909 says the annuitant can work?

If the physician indicates the annuitant can perform the duties of any occupation whose annual pay would exceed the earnings limit, the disability retirement benefit is terminated, and the WRS account is re-established as if no disability benefit had been received.
