Wisconsin ETF ET-5350: ICI Medical Report

Wisconsin state employees applying for Income Continuation Insurance need a physician to document their disability on this 2-page report under Wisconsin Statutes 40.61 and 40.62. The physician gives the diagnosis, objective findings, services since the last day worked, hospital stays and surgery, then answers whether the patient is totally disabled under the short-term definition and whether the disability is permanent for any occupation.

Who completes it

The claimant completes Section I, an authorization to release medical information, and the top of the back page. A physician completes and personally signs Section II; a podiatrist, dentist, NP, PA or psychologist may also complete it when acting under physician supervision.

Submitting and payment

How to submit it
Mail the completed, signed form directly to ETF in Madison; don't return it to the patient. ETF notes that timely submission is very important.
Who pays
Any cost for the report is the applicant's responsibility.

Good to know

  • For short-term ICI, totally disabled means being unable during the first 12 months to perform all essential duties of one's own occupation, supported by objective evidence such as blood tests, MRI or X-rays.
  • Attach copies of the medical histories, reports or notes behind your opinion; the 2011 form still labels the diagnosis code field ICD.9.
  • Physicians not licensed in Wisconsin must provide verification of their state licensing with the report.
  • The form must not be signed by an authorized representative or alternate provider; the 2011 edition names Aetna as claims administrator, which may since have changed.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Wisconsin ICI Medical Report?

A licensed medical doctor, doctor of osteopathy or surgeon, who must sign personally. A podiatrist, dentist, nurse practitioner, physician assistant or psychologist acting within their license may also complete it when supervised by a licensed physician; supervision isn't required for a DPM or DDS. The claimant cannot be the physician.

Should I give the completed ET-5350 back to my patient?

No. The instructions to the physician say to mail the completed and signed form directly to the Department of Employee Trust Funds and not to return it to the patient. The claimant's job is to complete Section I and bring the form to you promptly.
