OPERS DR-CMT: Disability Continued Medical Treatment

Ohio Public Employees Retirement System disability benefit recipients send this form every six months during the first three years after their benefit begins, unless they are in the Rehabilitative Services Program. The attending MD or DO reports treatment, findings by body system or mental status, and whether the recipient remains permanently disabled from their last public employment position.

Who completes it

The recipient completes Step 1, personal information. A licensed and practicing MD or DO completes Steps 2 through 6: physician information, patient information, the physician determination, physician findings and continued medical treatment.

Submitting and payment

How to submit it
The recipient submits the completed form to OPERS by fax or mail with the last six months of medical records. OPERS cannot accept digital media, so include report findings only.

Good to know

  • OPERS considers a member permanently disabled if the condition is expected to last at least 12 months and prevents the duties of the last public employment position.
  • Steps 1, 2 and 4 are required or the form is invalid; Step 4 asks for a prognosis and, if the member is not disabled, an expected return date or possible restrictions.
  • Attach all medical or psychological records from the last six months, including office notes, clinic and ER visits, labs, test results and discharge summaries.
  • Rehabilitative Services Program participants do not file DR-CMT, and initial applications use the DR-APS Report of Physician instead. Current revision 11/23.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often is the OPERS DR-CMT form required?

Every six months during the first three years after the disability benefit effective date, for recipients not enrolled in the Rehabilitative Services Program. After year three the form is no longer required, though OPERS still conducts periodic medical re-reviews every three years.

Can a nurse practitioner complete OPERS DR-CMT?

No. The physician steps must be completed by the member's attending physician who is a licensed and practicing MD or DO, and the physician marks MD or DO next to the signature in the determination step.

What records go with the DR-CMT?

All medical and psychological records from the last six months: office notes, clinic and ER visits, labs, all test results and discharge summaries. OPERS cannot accept digital media such as image discs, so the attachments should be report findings only.
