STRS Ohio Attending Physician's Report: Disability

Ohio teachers in STRS Ohio applying for a disability allowance or disability retirement need a report from their treating medical specialist on whether they are disabled from their last position for at least 12 months. The form is not online: it comes in the Disability Application Packet that STRS Ohio mails to members on request, and the member returns it with the rest of the packet.

Who completes it

The member's treating medical specialist, an MD or DO other than the primary care physician, completes the whole report after personally examining the member within two months of STRS Ohio's receipt of the report. The member completes the Disability Benefit Application and the employer completes the Report by Employer.

Submitting and payment

How to submit it
The physician seals the completed report and objective evidence in the physician envelope, and the member returns it with the application packet to STRS Ohio.
