Texas STD-27 Confidential STI Report

All Texas providers who diagnose or treat a reportable sexually transmitted infection must report it to the local health authority within 7 days, and the STD-27 is the DSHS form for doing so. It covers chancroid, gonorrhea, chlamydia, PID, lymphogranuloma venereum, syphilis by stage and HIV status, with test type and treatment. Disease Intervention Specialists use page 2 for confidential partner notification.

Who completes it

The diagnosing or treating provider completes both pages; shaded areas are not required by law but help identification and follow-up. Page 2 lists sexual or needle-sharing partners and the staff contact the health department should call first.

Submitting and payment

How to submit it
Mail it to the local health department or the DSHS HIV/STD Program within 7 days; DSHS lists local reporting authorities by region. Primary and secondary syphilis also need a phone call within one working day.

Good to know

  • Each disease block lists common regimens with set dosages, an 'Other' option and 'No Treatment Given', so record the treatment actually prescribed and its date.
  • When partners are notified of exposure, the Disease Intervention Specialist does not reveal the patient's identity.
  • Reporting HIV on the STD-27 counts as a timely report, but DSHS requires additional information on HIV patients through its HIV case report forms.
  • Laboratories use a different form, the STD-28, to report STD and HIV test results.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How soon must an STI be reported in Texas?

Within 7 days to the local health authority, as printed at the top of the STD-27. Primary and secondary syphilis need a phone call within one working day as well, with the STD-27 still due within seven calendar days. Laboratories have their own deadlines for reporting results.

Do I have to list partners on the Texas STD-27?

Page 2 asks for the patient's sexual or needle-sharing partners so a Disease Intervention Specialist can notify them confidentially. The form says the specialist will not reveal the patient's identity, and the provider can ask to be consulted before staff contact the patient.
